The Hospital Pharmacist will join an approximately 200–300 bed acute care hospital with physician-led governance and a strongly collaborative, interprofessional culture. The Hospital Pharmacist will support a facility accredited by The Joint Commission with recognized programs in stroke and cardiac care, comprehensive surgical services, and advanced critical care. The Hospital Pharmacist will contribute to a medication-use system enhanced by CPOE, barcode verification, smart pumps, and robust antimicrobial stewardship.
The Hospital Pharmacist reports to the Director of Pharmacy and partners closely with Clinical Pharmacy leadership. Pharmacy technicians, interns, and students align under the Hospital Pharmacist for day-to-day workflow oversight and coaching. The Hospital Pharmacist collaborates with hospitalists, intensivists, surgeons, emergency clinicians, nursing leadership, case management, and quality to drive safe and effective therapy.
The Hospital Pharmacist supports a centralized pharmacy model with decentralized rounding across ICU, ED, OR, and medical-surgical areas, serving an active census appropriate to a mid-size regional referral hospital. The Hospital Pharmacist works within a USP 797/800-compliant cleanroom, automated dispensing environments, and IV admixture services in a fast-paced yet supportive culture. The Hospital Pharmacist engages in order verification, clinical consults, transitions of care, and stewardship initiatives alongside experienced physicians and skilled pharmacy technicians.
The Hospital Pharmacist provides comprehensive pharmaceutical care through integrated drug distribution, clinical monitoring, patient counseling, and education. The Hospital Pharmacist verifies and fills medication orders, monitors drug therapies (including pharmacokinetics and renal/hepatic dosing), participates in interdisciplinary rounds, and delivers timely drug information to clinicians and patients. The Hospital Pharmacist supervises and directs supportive staff and may assume essential responsibilities of the Director of Pharmacy in the director’s absence, while also contributing to quality improvement, medication safety, formulary stewardship, policy development, and precepting.
